Garfield Park Behavioral Hospital offers no cost, confidential assessments for children and teens, 24 hours a day, 7 days a week. To arrange an assessment, call 773-265-3720 and ask to speak with an assessment and referral specialist. You can also visit the hospital anytime and request an assessment. No appointment or referral is necessary.

Master’s-prepared clinicians meet each patient and family to gather information about the child’s level of need, ability to be safe and any other concerns. Our assessment and referral staff are committed to patient-centered care and can provide assessments in the patient’s or family’s language of choice.

PATIENT CONFIDENTIALITY

The sensitive nature of personal and family issues demands protection of patient confidentiality. Only people you authorize will know about your child’s hospitalization. These people will only receive information necessary for them to continue care or provide support after discharge from the hospital.
